How CGMs Are Changing the Game for Fitness Professionals

Continuous glucose monitors (CGMs) have transformed how people with diabetes track their blood sugar, and that impact is now moving well beyond clinical care. For fitness and wellness professionals, including personal trainers, strength coaches, physical therapists, and health coaches, CGMs offer a window into a client's real-time metabolic response to training. Used within scope, that data helps professionals deliver more personalized, evidence-informed coaching.

What a CGM Actually Measures

A continuous glucose monitor is a small, wearable sensor that reads interstitial glucose levels every few minutes, generating a continuous stream of data rather than a single snapshot.1 Unlike a finger stick and meter, a CGM shows how glucose fluctuates throughout the day in response to meals, sleep, stress, and exercise.

Why This Matters in a Training Session

Both clients with diagnosed diabetes and clients without it can experience meaningful glucose swings during prolonged or intense exercise, sometimes showing up as dizziness, fatigue, or unusual burnout mid-session. For clients with Type 1 or insulin-dependent Type 2 diabetes specifically, hypoglycemia during exercise is a well-established medical concern.2 Many CGMs include customizable alerts that notify the wearer if glucose drops below a set threshold, such as 70 mg/dL, giving both the client and the coach earlier warning than symptoms alone would provide.

  • Reviews the client's target range and any low-glucose alerts before a session begins, when that information is available

  • Observes how glucose responds to different training intensities and adjusts pacing, rest, or session structure within the scope of exercise programming

  • Recognizes when a reading or trend falls outside what is appropriate to coach around, and pauses the session or refers the client to their physician following the Recognize and Refer protocol

  • Never recommends, adjusts, or comments on insulin dosing, medication timing, or any other treatment decision, those decisions belong exclusively to the client's physician or diabetes care team

Showing Clients Their Progress

One of the most useful coaching applications of CGM data is visibility. A coach can show a client objective markers of change over time, such as an improving time-in-range percentage across a training block, giving the client tangible evidence that the process is working, alongside the usual strength and body composition markers.

Where the Line Stays Firm

CGM data is powerful, but it does not expand a fitness professional's scope of practice. A trainer, strength coach, physical therapist, or health coach observes patterns and adjusts training variables within their existing scope. Any question involving medication, insulin dosing, or diagnosis stays with the client's physician, full stop.
